What to Bring

For your first visit, please bring a valid photo ID, your insurance card, a list of current medications with dosages, any recent lab results or imaging reports, and the name and contact of your primary care physician. Arrive 15 minutes early to complete registration paperwork.

Office Hours

We are open Monday through Friday, 8:30 AM to 5:00 PM. Evening appointments are available on select Tuesdays by request. Walk-ins are welcome during business hours but appointments are recommended to minimize your wait time.

Our office is located at 114 Edwards St, Princeton, KY 42445, right in the heart of downtown. Free parking is available in the lot behind the building and along the adjacent street. The clinic is easily accessible and wheelchair-friendly.
